If your child or your family needs our help, all you have to do is ask
Often referrals are made by consultants, GPs, care professionals and social workers, but family members and carers can refer children to us directly too.
Complete one of our referrals forms to help us get a better understanding of the family's situation. This will help us make sure a child and their family gets exactly the right support for them.
Once we receive your referral and we confirm it meets our eligibility criteria, we'll arrange to visit you the child and their family at home to carry out an initial assessment and talk about the support they need. We'll then discuss it at our next weekly multi-professional panel meeting.
If the child you're referring does not meet our eligibility criteria, we will call you to let you know and we will confirm that decision in writing to you and the professionals involved in your child’s care. We will also try to signpost you to other services which may be able to offer help or support for the child and their family.
